   1. Booking Certificate / Travel Itinerary
   1. Hotel Confirmation/Voucher
   1. Authenticated Birth/Marriage Certificate from PSA (Formerly NSO) issued within one year

      1. PERSONAL APPEARANCE BY APPOINTMENT

      1. PROOF OF ASSETS LIKE CAR REGISTRATION AND LAND TITLES, IF ANY

Minor Applicants: School registration/proof of enrollment
