4. Employment Certification (Please state position and salary)
5. Approved leave of absence (Employed)
6. Copy of Latest Income Tax Return (ITR) with BIR Stamp; For Self-employed ITR must include pages with Net income and total revenue
7. Original Bank Certification and copy of latest bank statements for the past 6 months 
8. Detailed Day to Day Itinerary 
9. Flight Itinerary 
10. Hotel Confirmation or Reservation 
11. Travel Insurance – with Php2.5M coverage; covering the whole duration of the trip **(+15 Days coverage for ITALY and AUSTRIA)**
12. PSA (Formerly NSO) Certified Marriage Certificate of married applicants 
13. PSA (Formerly NSO) Certified of minor applicants
14. School registration/proof of enrollment of minor applicants
15. Affidavit of Support and Consent if minor is not travelling with either or both parents
16. FOR BUSINESS TRIP: Basic requirements for Tourist Visa + Endorsement letter from the Employer & Invitation from counterpart abroad
17. Personal appearance for Biometrics (By appointment)
