REQUIREMENTS FOR TOURIST

  1. Original valid passport – including old passport with previous visa to Japan (if any) Must be 6months valid
  2. Accomplished Visa Form (A4) Size paper (Click here)
  3. 2 pcs Passport Size Photo 45mm x 35mm colored photos (white background)
  4. Original Employment Certification (Please state position, date hired and salary in Company letter head)
  5. Copy of Business Registrations – for Self-employed applicants
  6. Original Bank Certification (Issued within 1 month of application)
  7. Copy of Latest INCOME TAX RETURN (ITR) Form or 2316 with corresponding payment receipt from BIR authorized bank (Employed)
  8. ITR Self-Employed F-2316 / F-1701 ITR should include the pages with net income and total revenue
  9. Day to day Itinerary/Activity in Japan
  10. Original Birth Certificate from PSA (Formerly NSO) Issued within one year (applicants without previous Japanese visa)\
  11. Original Marriage Certificate (Female Applicants) from PSA (Formerly NSO) issued within one year (applicants without previous Japanese visa)

Minor Applicants: School registration/proof of enrolment/ School Certification

                                Notarized Affidavit of Support and Consent if not travelling with either or both parents
