REQUIREMENTS FOR NANNY/PERSONAL ASSISTANT/PERSONAL CAREGIVER

  1. Computerized and Signed Application Form (A4 Size Bond Paper)
  2. 1 piece of Passport size colored picture (Please attach on the application form)
  3. Original Passport (should be valid for more than 6 months)
  4. Photocopy of Passport Bio-page (page 2) (A4 Size Bond Paper)
  5. Original & Photocopy of valid and previous visa/s and arrival stamps for the past 5 years
    1. If applicable only; Must show proof of arrival stamp or slip
  6. Employer’s Employment/Business Documents
    1. If EMPLOYED
      1. Employment Certificate Original [must include position, date hired, compensation, office address, 

HR landline number (cell-phone number is not allowed), HR e-mail address] (Issued within 1 month of application)

  1. Photocopy of ITR (Income Tax Return) or Form 2316 Copy
    1. If SELF-EMPLOYED
      1. Business Registration from SEC or DTI (Copy), Business Permit or Mayor’s Permit (Copy)
      1. Photocopy of ITR (Income Tax Return) or Form 2316 / 1701 with corresponding payment receipt from

BIR authorized bank and should include the pages with Net income and Total revenue

  • Employers Bank Documents
    • Latest Original Personal Bank Certificate (Issued within 1 month of application
      • Must include account type, current balance, account opening date, ADB)
    • Latest Original Bank Statement (3 months) (Issued within 1 month of application)
      • Original or certified true copy of bank statements/passbook for the last 3 months
  • CONTRACT signed Helper and Employer: Must include position, date hired, compensation, scope of work and day-off.
  • Notarized Affidavit of Support by the Employer
  • COVER LETTER: Execute a computerized, English, and signed letter of explanation for documents you cannot provide

■ Processing Period
-Visa processing and releasing time is minimum of 5-7 working days from the scheduled appointment
-Requirements must be completed and submitted immediately or at least 2-3 weeks before the scheduled slot.
