REQUIREMENTS FOR INVITED BY MILITARY PERSONNEL BASED IN JAPAN

1. Original valid passport – including old passport with previous visa to Japan, if any
2. 2 pcs Passport Size 45mm x 35 mm colored photos (white background)
3. Accomplished Visa Form (Click here)
4. Proof ofRelationshipsuch as pictures, passport copies etc.

For visiting friends/acquaintances: Birth Certificates and Marriage Certificate to connect relationship for visiting relatives.

5. Original Latest Income Tax Return (ITR) Original Daily Schedule in Japan (TAIZAI YOTEIHYO)
6. Original Statement of Service – If possible, please submit statement of service with name and email address of attestant.
7. Passport Copy of US Military Personnel in Japan (Information page and pages with stamps regarding Japan travel)
8. Original Invitation Letter
9. Original Bank Certificate Validity of Bank Certificate is three (3) months from the date of issuance
10. Income Tax Return (Form 2316) clear Photocopy
11. Birth Certificate from PSA (Formerly NSO) issued within one year (applicants without previous Japanese visa)
12. Marriage Certificate (Female Applicants) from PSA (Formerly NSO) issued within one year (applicants without previous Japanese visa)
13. Original Letter of Authorization for Embassy Verification of PSA Documents (Click here)
