3. Employment Certification / Business Registrations for Self Employed
4. Copy of Latest Income Tax Return (ITR) with BIR Stamp; For Self-employed ITR must include pages with Net income and total revenue
5. Endorsement Letter from Employer (BUSINESS VISA)
6. Copy of Business Registrations of Employer (BUSINESS VISA
7. Bank Certification of applicant – with bank statements for the past six months
8. Invitation or Endorsement Letter from host (BUSINESS VISA)

12. Personal appearance of the applicant is required for Biometrics
