  1. Original valid passport – Including old passport with previous visa within 10 years
  2. 2 pcs. passport-sized photo (coloured/white background)
  3. Accomplished visa form
  4. Employment Certification (Please state position and salary)
  5. Copy of Business Registrations (Self-employed)
  6. Approved leave of absence (Employed)
  7. Copy of Latest Income Tax Return (ITR) with BIR Stamp; For Self-employed ITR must include pages with Net income and total revenue
  8. Original Bank Certification with Bank statements for the past 6 months
  9. Flight Reservation/Confirmation/Detailed Itinerary/Cruise Confirmation/Onward Visa
  10. Personal Appearance for biometrics
  11. PSA Birth / Marriage Certificate

Minor Applicants: School registration/proof of enrollment
PSA (Formerly NSO) Birth Certificate
Affidavit of Support and Consent if not travelling with either or both parents
