  1. Computerized and Signed Application Form (A4 Size Bond Paper)
  2. 1 piece of Passport size colored picture (Please attach on the application form)
  3. Original Passport (should be valid for more than 6 months)
  4. Photocopy of Passport Bio-page (page 2) (A4 Size Bond Paper)
  5. Copy of Seaman’s book
  6. Copy of Medical Certificate
  7. Copy of OEC Receipt & Copy of Overseas Employment Certificate(OEC)\
  8. Original Guarantee Letter from Manning / Shipping Company in the Philippines (No given format)
  9. Copy of 2 Guarantee Letters from Korean Company (Refer to attached Embassy formats)
  10. Copy of Korean Company business permit
  11. COVER LETTER: Execute a computerized, English, and signed letter of explanation for documents you cannot provide

■ Processing Period
-Visa processing and releasing time is minimum of 5-7 working days from the scheduled appointment
-Requirements must be completed and submitted immediately or at least 2-3 weeks before the scheduled slot.
