1. Computerized and Signed Application Form (A4 Size Bond Paper)
2. 1 piece of Passport size colored picture (Please attach on the application form)
3. Original Passport (should be valid for more than 6 months)
4. Photocopy of Passport Bio-page (page 2) (A4 Size Bond Paper)
5. Original & Photocopy of valid and previous visa/s and arrival stamps for the past 5 years
   1. If applicable only; Must show proof of arrival stamp or slip
6. Copy of Invitor’s(Korean/Filipino) Passport Bio page (or valid ID)
7. Invitor’s (Korean/Filipino) Invitation letter (Designated format from Embassy or Hikorea Website)
8. Invitor’s (Korean/Filipino) Letter of Guarantee (Designated format from Embassy or Hikorea Website, guarantee period must be 3 years from entry date) 
9. Invitor’s Letter of Pledge on Illegal stay and Unemployment (Designated format from Embassy or Hikorea Website)
10. Residential Requirement document of the Korean (Real estate register, Contract of lease)
11. Letter of Pledge on Unemployment (Designated format from Embassy or Hikorea Website)
12. Medical Certificate on tuberculosis (issued from hospitals designated by the Korean embassy within 3months)
13. Invitor’s Korean Basic Certificate (detailed, issued within 3 months) 
14. Invitor’s Korean Family Certificate (detailed, issued within 3 months) 
15. Invitor’s Korean Marriage History (detailed, issued within 3 months) 
16. Korean Resident Registration Certificate (issued within 3 months) 
17. Korean Family Certificate under Korean’s child (detailed, issued within 3 months) (If pregnant, Pregnancy Certificate, or Maternity Handbook)

※  If there is more than one child, Korean Family Certificates of all children must be submitted. If child is adopted, Korean Adoption Certificate must be submitted

1. Proof of Philippine family relation documents such as PSA Birth Certificate and PSA Marriage Certificate
