REQUIREMENTS FOR CHILDREN OF A FILIPINO MARRIED TO MILITARY PERSONNEL STATIONED IN KOREA

1. Computerized and Signed Application Form (A4 Size Bond Paper)
2. 1 piece of Passport size colored picture (Please attach on the application form)
3. Original Passport (should be valid for more than 6 months)
4. Photocopy of Passport Bio-page (page 2) (A4 Size Bond Paper)
5. Original & Photocopy of valid and previous visa/s and arrival stamps for the past 5 years
   1. If applicable only; Must show proof of arrival stamp or slip
6. Photocopy of PSA Birth Certificate of the Applicant
7. Invitation Letter
8. Photocopy of Inviter’s Passport Bio-page (page 2)
9. Photocopy of Inviter’s Military ID (Front & Back)
10. Photocopy of Military Order
11. Photocopy of Filipino Parent’s Passport Bio-page (page 2) and Visa page
12. Photocopy of Filipino Parent’s PSA Marriage Certificate
13. Parents Bank Certificate [must include account type, current balance, account opening date, 6 months average balance (ADB)] (issued within 30 days)
14. Parents Bank Statement (original or certified true copy of bank statements/passbook for the last 3 months)
15. COVER LETTER: Execute a computerized, English, and signed letter of explanation for documents you cannot provide

* IF INVITED BY COMPANY IN KOREA

-Photocopy of Korean Company Business Permit (authentication not required)

■ Processing Period
-Visa processing and releasing time is minimum of 5-7 working days from the scheduled appointment
-Requirements must be completed and submitted immediately or at least 2-3 weeks before the scheduled slot.
