  2. 2 pcs. passport-sized photo (colored/white background)
  3. Accomplished visa forms (Click here)
  4. Employment Certification (Please state position and salary)
  5. Copy of Business Registrations (Self-employed)
  6. Approved leave of absence (Employed)
  7. Copy of Latest Income Tax Return (ITR) with BIR Stamp; For Self-employed ITR must include pages with Net income and total revenue
  8. Original Bank Certification with Bank statements for the past 6 months
  9. Flight Reservation/Confirmation/Detailed Itinerary
  10. Personal Appearance for biometrics at VFS
  11. FOR INDIAN NATIONAL: SAME REQUIREMENTS WITH FILIPINO + ACR-I CARD

Minor Applicants:

School registration/proof of enrollment

PSA (Formerly NSO) Birth Certificate

Affidavit of Support and Consent if not travelling with either or both parents
